The FDA updated its CORE Investigations of Foodborne Illness Outbreaks table on October 28, 2026. Here is what’s new:

New outbreak: A new outbreak of Salmonella Javiana (ref #1424) has been added to the table. Fifty-nine people are sick. The product linked to illnesses has not yet been identified, and FDA has not yet initiated traceback, an inspection or sampling. The outbreak is ongoing.

Listeria monocytogenes (ref #1417): The case count has increased from 46 to 50. The source has not been identified. FDA has initiated traceback, an inspection and sampling, and the outbreak is ongoing.

Salmonella Javiana (ref #1395), jalapeño peppers: FDA’s outbreak investigation has closed. FDA updated the advisory with final information about the outbreak investigation on October 2, 2026.

Cyclospora (ref #1384), product not identified: The Executive Incident Summary Abstract can now be accessed by clicking the reference number on the FDA table. This outbreak investigation was moved to the Closed 2026 Table.

E. coli O157:H7 (ref #1421, USDA 2026-04), multi-ingredient wraps with chicken: The outbreak, which sickened 50 people, has ended and there is no ongoing risk to consumers. According to FDA, the wrap identified through the investigation is past shelf life and no longer available for purchase, and the implicated wraps were proactively removed from the market before the link to the outbreak was established. At that time, illnesses had occurred during one week in August. FDA says that because the products were removed from sale and the wraps people ate were out of shelf life, more detailed product information would not have given consumers an action they could take to prevent illness. FDA’s outbreak investigation remains active.

By the numbers: There are now 15 active investigations on the CORE table. Six outbreaks are ongoing and nine have ended. Ten of the 15 have no product yet identified. The outbreaks with published case counts total at least 743 illnesses, and that does not include the outbreaks where FDA refers readers to an advisory: E. coli O26:H11 linked to raw milk cheese (ref #1419), Salmonella Bovismorbificans linked to sprouts (ref #1411), E. coli (multiple strains) and Salmonella Agona linked to alfalfa sprouts (ref #1394), and Cyclospora linked to iceberg lettuce (ref #1390).

Unsolved outbreaks still on the table: Salmonella Javiana (#1424, 59 sick, ongoing); Listeria monocytogenes (#1415, 11 sick, ongoing); Listeria monocytogenes (#1417, 50 sick, ongoing); Salmonella Newport (#1403, 119 sick, ended); Salmonella I 4,[5],12:i:- (#1402, 43 sick, ended); Cyclospora (#1398, 22 sick, ended); Cyclospora (#1392, 241 sick, ended); Listeria monocytogenes (#1379, 11 sick, ended); Cyclospora (#1385, 38 sick, ended); and Salmonella Oranienburg (#1387, 99 sick, ended).
